Patient Forms

Upon entering our office our receptionist will greet you and welcome you as a member of our family.  We will request you fill our patient forms located in the New Patient Center. The paperwork provides us with general information about yourself and your condition.


Consultation

Consultation

 

A majority of our patients will have a consultation with Dr Alan or Dr. Tom, unless directed otherwise by a script from another referring practitioner. Your doctor and/or practitioner will discuss your health related problems and concerns as well as potential treatment options. Your first visit is designed for the doctor or practitioner to learn more about you, your condition and your expectations to determine the most effective treatment plan for your needs. Subsequent consultations may be set up with additional practitioners initially or as you progress throughout your prescribed treatment plan.


Examination

When chiropractic care and complete wellness programming is recommended,  Dr. Alan or Dr. Tom will perform a complete panel of neurologic, orthopedic, postural and physical examinations in order to fully assess your condition, identify the cause of that condition, and then develop a complete treatment plan to correct the condition. Nothing will be done in our office without your consent.

 

When physical therapy, corrective exercise, and or at home exercise program development are recommended, a physical therapist or corrective exercise specialist will perform a complete series of tests to assess the structure, function and balance of your neuromuscular system. A complete exam will include full history, postural analysis, movement assessment, muscle length testing, and muscle activation testing.

 

When nutritional counseling is recommended, a patient will be referred to one of our nutritional counselors for a complete analysis of food choices, nutritional deficiencies, shopping habits, and dietary supplementation plan. Once all information is gathered and a week-long food journal is completed, the counselor and you meet to develop attainable nutrition goals, and dietary guidelines in order to address the areas of weight loss, body composition, correction of systemic deficiencies, and healthy eating in a variety of lifestyle situations.

Subluxation Station

To give you the patient the best treatment, we need to have the best diagnostic equipment, which we feel we have here at Pivotal Wellness. Besides our extensive skills in orthopedic, neurological and sensory testing, we use the Insight Millennium, which is certified space technology, by the Space Foundation. The Insight Millennium is an electronic neurological testing station, which gives us insight to your nervous system. This screening and evaluation system that uses surface electromyography technology (SEMG) , was originally designed to measure changes in the spinal muscles of astronauts.


Report of Findings

Once all the information and examinations have been performed, the doctor will give you a report of findings to answer the three most popular questions: 

  1. Can you help me?
  2. What do you recommend that I do to get better?
  3. How much is it going to cost?

After discussing with you your history and your goals, examining your spine and reviewing your scan, the doctor will discuss recommendations with you. If you have a condition that requires care with other providers, the doctor will tell you. If Dr. Alan or Dr. Tom believes we can help you, we will recommend treatment and a wellness program.


Treatment

Chiropractic

 

Patients will generally be provided with their first chiropractic treatment at the conclusion of the appointment with a chiropractor. This may include spinal adjustments, physical therapies, and or soft tissue therapies such as cold laser therapy. Treatment is always interactive with you so you can express any concerns you have about the style of adjustment you will be most comfortable with.

 

Physical Therapy & Exercise Therapy

 

Corrective exercise treatment plans are developed from initial diagnostic findings and progression of care and wellness programming.  Once a coordinated treatment is developed, the practitioner may coach you through soft tissue release, muscle activation, flexibility and strength training and core stabilization with the goal being to restore muscle balance, reduce joint stress, and retrain the body to function as is should, thereby reducing pain and dysfunction.

 

Massage Therapy

 

Massage therapy treatment plans are developed from initial diagnostic findings, progression of care and wellness programming. Our massage therapists focus on the release and activation of the overactive and underactive muscles around an affected joint focusing on resorting muscle balance and joint coordination. This is done through trigger point release, active release techniques, myofascial release, neuromuscular reeducation, Swedish massage, essential oil treatments, energy balancing, and shiatsu massage.

 

Nutritional/Supplementation Programming

 

Nutritional and supplementation programming is developed from a number of areas ranging from lifestyle analysis such as food journals, shopping lists, and eating behaviors to clinical panels such as blood, urine, saliva. Once the strengths and weaknesses of your diet are assessed, our nutritionists and chiropractors can begin to develop a plan to address conditions ranging from weight loss to fatigue to blood sugar regulation to metabolic syndrome.

As mentioned in our treatment descriptions, we strive to educate our patients on independent wellness care that will continue their progress during clinical care.

 

At Home Exercise Programming

 

The main objective of our physical therapy, corrective exercise, and functional fitness programming is training the body to heal the body. However, re-training the body to function correctly requires practice.  In addition to your time spent in the clinic being coached by your therapist, you will have progressive independent exercise programs developed to guide practice of corrective exercises and development of physical fitness while reinforcing stability and continued improvement in areas of imbalance or dysfunction.  Our goal is to educate you how to exercise correctly, play safely, recognize causes of pain, and have an action plan to reduce the stimulus for pain or dysfunction effectively.

 

Nutritional Programming

 

How we fuel our body determines how we think, feel, and function throughout our activities of daily living. The focus of our nutritional programming at Pivotal Wellness is to educate, guide, and empower you to develop healthy habits and make lifestyle choices necessary to heal the body and achieve desired goals for health and wellness. Our nutritional programming is supported by a foundation of healthy lifestyle habits including:

 

1. Food journaling & food choices

2. Shopping strategies & label reading

3. Eating behaviors (volume & quality coinciding with life events/stress)

4. Physiologic health (is the body balanced, what supplementation is needed)

5. Patient Lifestyle Goals

 

As data from these areas is collected and analyzed, a nutritionist and you work to develop a nutritional and supplementation plan that addresses your physiologic needs as well as personal lifestyle goals. Most importantly, we develop goal setting, wellness programming, and life changes that are realistic and feasible for any type of career path, family life, or personal goal. 

 

 

Supplementation

 

The right supplements can be an important part of an overall wellness program designed to address any specific health problems or concerns you may have. However, not all supplements are created equal. When it comes to protecting your health, quality should be your number one priority. Simply, the quality of a supplement dictates its effectiveness. You should also know that nutritional supplement manufacturers are not held to the same standards as pharmaceutical companies. There are a lot of “fly-by-night” operations offering cheap supplements of questionable quality. So how do you know what is which products are of the highest purity, consistent quality, and effectiveness?

 

All of Pivotal Wellness’ supplement lines are at least GMP (Good Manufacturing Practices) certified. This means that third-party companies focused on supplement quality, such as Natural Nutritional Foods Association and NSF International, routinely audit supplement companies to ensure purity and consistent quality. Such supplements may also be deemed medical foods as they are formulated and tested to treat clinical conditions.
